The contract shall specify :
1° The educational qualifications and professional qualifications ;
2° The provisions of article R. 6152-402 under which the recruitment is made;
3° The nature of the duties performed as well as the service obligations incumbent on the practitioner, in particular with regard to his participation in the continuity of care or pharmaceutical duty on site or on call;
4° The date on which the practitioner takes up his post, the duration of the contract and the date on which it ends and, where applicable, the trial period, set at one month for a contract lasting less than six months and two months for a contract lasting six months or more;
5° The period of notice in the event of early termination of the contract or resignation, i.e. one month for a contract of less than six months and two months for a contract of six months or more;
6° Indication of the social protection scheme (general social security scheme and Ircantec supplementary pension scheme);
7° Remuneration set in accordance with the recruitment conditions set out in articles R. 6152-402 and R. 6152-403 and the provisions of article R. 6152-416 , as well as any additional allowances.
Renewal of the contract may be agreed in the form of an amendment to the initial contract, where the reason for recruitment remains unchanged.
