The request for an increase must be made in the following form and under the following conditions:
1° The request is sent by registered letter with acknowledgement of receipt to the chairman of the Enforcement Committee;
2° In the case of natural persons, the application shall state the surname, first name, profession and domicile of the applicant; in the case of legal entities, the application shall state their name, legal form and registered office as well as the body that legally represents them;
3° The following are attached
a) A copy of the decision imposing the permanent ban on practising or the permanent withdrawal of the professional card whose lifting is requested and, where applicable, a copy of the decisions of the appeal courts;
b) A detailed statement of the reasons justifying the request for removal;
c) Where necessary, the documents that the applicant wishes to invoke in support of his application.
