Article R6222-3 of the French Public Health Code

On all professional titles and documents, in particular on all reports issued by the medical biology laboratory, the following information must be clearly displayed:

1° The name or business name of the medical biology laboratory and its form of operation;

2° The name of the biologist in charge of the medical biology laboratory and, where applicable, the names of the biologists in charge;

3° The accreditation number where full accreditation has been granted.

In addition, the address of the site or sites involved in the examination is given on each examination report.

Each site of a medical biology laboratory is identified to the public by a professional plaque affixed to the entrance door of the site and, where applicable, to the door of the building in which the site is located. This plaque may not contain any information other than that mentioned in 1° and 2°.

Sur tous les titres et documents professionnels, notamment sur tous les comptes rendus émanant du laboratoire de biologie médicale, figurent de façon apparente les mentions suivantes :

1° Le nom ou la raison sociale du laboratoire de biologie médicale et sa forme d’exploitation ;

2° Le nom du biologiste-responsable du laboratoire de biologie médicale et, le cas échéant, le nom des biologistes-coresponsables ;

3° Le numéro d’accréditation dès qu’une accréditation totale est délivrée.

En outre, sur chaque compte rendu d’examen, figure l’adresse du ou des sites concernés par la réalisation de l’examen.

Chaque site d’un laboratoire de biologie médicale est signalé au public par une plaque professionnelle apposée à la porte d’entrée du site et, le cas échéant, à la porte de l’immeuble dans lequel est installé le site. Cette plaque ne peut comporter d’autres indications que celles mentionnées aux 1° et 2°.
