The application for authorisation or renewal of authorisation submitted by an employer with a view to hiring a child to carry out the activity mentioned in 5° of article L. 7124-1 must be accompanied by the following documents:
1° A birth certificate for the employer or its directors, partners and managers in the case of a company;
2° A certificate of payment of contributions to social security organisations for employers in activity at the time the application for approval is submitted;
3° A certificate in which the employer undertakes to have the child it employs undergo, at its own expense, the medical examination provided for in article R. 7124-19-1;
4° Any information enabling the employer’s character to be assessed, as well as the conditions under which he will be carrying out his activity;
5° A document establishing the child’s civil status;
6° Written authorisation from the child’s legal representatives, accompanied by a list of jobs previously or currently held by the child;
7° Any documents enabling the difficulties and character of the role that the child will be playing to be assessed;
8° Any details of the child’s employment conditions, remuneration and arrangements for school attendance.
