The application for approval or renewal of approval submitted by a modelling agency with a view to hiring children to carry out the activity referred to in 3° of Article L. 7124-1 must be accompanied by the following documents:
1 A birth certificate for the directors, partners and managers of the agency;
2 A certificate of payment of social security contributions for agencies in operation at the time the application for authorisation is submitted;
3 A certificate stating that the agency undertakes to have the child undergo the medical examination provided for in article R. 7124-9 at the agency’s expense;
4 A copy of the information sheet provided for in article R. 7124-15 ;
5 All the information needed to assess :
a) The character, competence and professional experience in the employment of child models of the directors, partners and managers of the modelling agency;
b) The financial situation of the agency, if it is in operation when the application is submitted;
c) The operating conditions of the agency, in particular with regard to the equipment it has, the premises in which it is located, and the number and qualifications of the staff employed;
d) The conditions under which it will operate with children.
