Article R7421-1 of the French Labour Code

At least two copies of the form or booklet given to the homeworker in application of article L. 7421-2 must be produced.
It mentions :
1° The name and address of the establishment or the name, first names and address of the employer;
2° The references of the organisations to which the employer pays social security contributions and the registration number under which these contributions are paid;
3° The registration number in the trade register or the register of trades;
4° The nature and quantity of the work, the date on which it is carried out, the time taken to carry it out, the custom prices or wages applicable;
5° The nature and value of the supplies required of the worker as well as the workshop and ancillary costs;
6° Where applicable, the date on which the work is delivered.

Le bulletin ou le carnet remis au travailleur à domicile, en application de l’article L. 7421-2, est établi en deux exemplaires au moins.
Il mentionne :
1° Le nom et l’adresse de l’établissement ou les nom, prénoms et adresse du donneur d’ouvrage ;
2° La référence des organismes auxquels le donneur d’ouvrage verse les cotisations de sécurité sociale et le numéro d’immatriculation sous lequel ces cotisations sont versées ;
3° Le numéro d’inscription au registre du commerce ou au registre des métiers ;
4° La nature et la quantité du travail, la date à laquelle il est donné, les temps d’exécution, les prix de façon ou les salaires applicables ;
5° La nature et la valeur des fournitures imposées au travailleur ainsi que les frais d’atelier et accessoires ;
6° Le cas échéant, la date à laquelle le travail est livré.
