The document referred to in 2° of Article D. 3111-6 contains the following details: 1° Full name, date of birth and address of the person vaccinated ; 2° Medical examinations and, where appropriate, biological tests carried out prior to vaccination; 3° Date of these examinations, date of vaccination; 4° Vaccine batch number and name of manufacturer; 5° Name and address of the vaccinator; 6° Date and signature of the vaccinator.
