Subsection 1: Company file.

For each company or establishment, the occupational physician or, in inter-company occupational health and prevention services, the multi-disciplinary team draws up and updates a company or establishment sheet listing, in particular, the occupational risks and the number of employees exposed to them.

For companies which are members of an inter-company occupational health and prevention service, the company form is drawn up in the year following the company’s or establishment’s membership of this service.

The company file is sent to the employer. It is presented to the Social and Economic Committee at the same time as the annual report provided for in Article L. 4612-16.

The company file is kept at the disposal of the regional director of companies, competition, consumption, work and employment and the medical labour inspector. It may be consulted by officials from the prevention departments of social security bodies and by officials from the professional bodies for health, safety and working conditions mentioned in article L. 4643-1.

The model company sheet is set by order of the Minister for Employment.
