A discharge form is given to patients before they leave the unit. This report, signed by one of the unit doctors, mentions the identity of the medical staff who took part in the operation, the recommendations on what to do in terms of post-operative or post-anaesthetic monitoring, particularly with regard to pain management, and the contact details of the health establishment staff responsible for continuity of care.

I.-The medical staff required for surgical activity includes: 1° Doctors specialising in surgery, whose speciality is adapted to the specific therapeutic practices mentioned in II of article R. 6123-202 and implemented by the holder of the authorisation for surgical care activity; 2° Doctors specialising in anaesthesia and intensive care. The authorisation for surgical care is only granted if the holder organises the surgical management of patients referred to it by emergency medical facilities, in accordance with 5° of article D. 6114-3.

The holder of the authorisation for the surgical care activity provides the operational directory of resources mentioned in 1° of article D. 6124-25 with updated information relating to its available and mobilisable resources.

In the context of exposure to ionising radiation, the authorisation holder shall ensure that staff and patients have access to the tools needed to optimise radiation protection.

The authorisation holder is subject to the obligation of quality assurance defined in I of Article L. 1333-19. The authorisation holder shall ensure that the equipment used to expose patients to ionising radiation is connected to a system for the systematic collection and archiving of dosimetric data. The authorisation holder shall ensure that data from professional practices is collected and analysed with a view to improving practices and managing risks. The medical team mentioned in 1° of the I of article D. 6124-271 fills in the professional practice observation registers mentioned in 3° of article D. 4021-2-1, once these registers have been set up.
