Article 1091 of the French Code of civil procedure

On pain of inadmissibility, the petition shall include in its appendix, where applicable, the information form of the minor child requesting to be heard dated and signed by him or her as well as an agreement dated and signed by each of the spouses and their lawyer settling in full the effects of the divorce and including in particular a liquidation statement of the matrimonial property regime or the declaration that there are no grounds for liquidation. The liquidation statement must be drawn up in notarised form when the liquidation involves property subject to land registration.

A peine d’irrecevabilité, la requête comprend en annexe, le cas échéant, le formulaire d’information de l’enfant mineur demandant à être entendu daté et signé par lui ainsi qu’une convention datée et signée par chacun des époux et leur avocat portant règlement complet des effets du divorce et incluant notamment un état liquidatif du régime matrimonial ou la déclaration qu’il n’y a pas lieu à liquidation. L’état liquidatif doit être passé en la forme authentique devant notaire lorsque la liquidation porte sur des biens soumis à publicité foncière.
