Article 1132 of the French Code of civil procedure

In the event of legal separation by mutual consent, and where a minor applies to be heard by the judge, the application for conversion to divorce by mutual consent based on article 230 of the Civil Code shall contain, on pain of inadmissibility, the particulars required by article 1090, an indication of the decision that pronounced the legal separation, and is accompanied by an agreement on the consequences of the divorce. Under the same sanction, the petition and the agreement are dated and signed by each of the spouses and their lawyer.

En cas de séparation de corps par consentement mutuel, et lorsqu’un mineur demande son audition par le juge, la requête aux fins de conversion en divorce par consentement mutuel fondée sur l’article 230 du code civil contient, à peine d’irrecevabilité, les mentions requises par l’article 1090, l’indication de la décision qui a prononcé la séparation de corps, et est accompagnée d’une convention sur les conséquences du divorce. Sous la même sanction, la requête et la convention sont datées et signées par chacun des époux et leur avocat.
