Article 1149-1 of the French Code of civil procedure

Where, in the case of a change of filiation, the child of full age consents to the change of his or her name, this consent is received by a civil registrar, a notary, a French diplomatic or consular agent or by the court which pronounces the legitimation; in the latter case, mention of it is made in the operative part of the decision.

Lorsque, en cas de changement de filiation, l’enfant majeur consent à la modification de son nom, ce consentement est reçu par un officier de l’état civil, un notaire, un agent diplomatique ou consulaire français ou par la juridiction qui prononce la légitimation ; dans ce dernier cas, il en est fait mention au dispositif de la décision.
