Article 1218 of the French Code of civil procedure

The application for the issue of a protective measure for a person of full age shall include, on pain of inadmissibility:

1° The detailed medical certificate provided for in article 431 of the Civil Code;

2° The identity of the person to be protected and a statement of the facts requiring this protection under articles 428 and 494-1 du même code.

Article 1218

La requête aux fins de prononcé d’une mesure de protection d’un majeur comporte, à peine d’irrecevabilité :

1° Le certificat médical circonstancié prévu à l’article 431 du code civil ;

2° L’identité de la personne à protéger et l’énoncé des faits qui appellent cette protection au regard des articles 428 et 494-1 du même code.
