For the implementation of the future protection mandate established pursuant to the third paragraph of Article 477 of the Civil Code, the mandatary shall appear in person at the registry of the judicial court in whose jurisdiction the beneficiary of the mandate resides, accompanied by the latter, unless it is established, by medical certificate, that his presence at the court is incompatible with his state of health.
The mandatary shall present to the court clerk:
1° The authentic copy of the mandate, signed by the mandator and the mandatary;
2° A death certificate of the mandator or a medical certificate no more than two months old, from a doctor on the list mentioned in article 431 of the Civil Code and establishing that the principal is in one of the situations provided for in article 425 of the same code ;
3° A medical certificate no more than two months old, issued by a doctor on the list referred to in article 431 of the Civil Code and establishing that the principal’s adult child designated as the beneficiary of the mandate is in one of the situations provided for in article 425 of the same code;
4° An identity document relating respectively to the mandatary and the beneficiary of the mandate;
5° Proof of the habitual residence of the beneficiary of the mandate.
