The attestation shall contain an account of the facts that the author witnessed or personally observed.
It shall mention the author’s surname, forenames, date and place of birth, place of residence and profession.
It mentions the surname, first names, date and place of birth, place of residence and profession of its author as well as, if applicable, his relationship of kinship or marriage to the parties, subordination to them, collaboration or community of interest with them.
It shall also state that it has been drawn up with a view to its production in court and that its author is aware that a false attestation on his part exposes him to penal sanctions.
The attestation must be written, dated and signed by its author. The author must attach to it, in original or photocopy, any official document proving his identity and bearing his signature.
