In addition to the information prescribed by articles 54 and 57, the application must contain, on pain of nullity, a summary statement of the grounds for the application. The documents that the claimant wishes to invoke in support of his claims shall be attached to his application in as many copies as there are persons whose summons is requested.
Where applicable, the application shall mention the applicant’s agreement to the proceedings being conducted without a hearing pursuant to Article L. 212-5-1 of the Code of Judicial Organisation.
When the application is filed electronically, the documents are attached in a single copy.
Where each party is represented by a lawyer, the application shall contain, on pain of nullity, the constitution of the lawyer or lawyers of the parties. It shall be signed by the lawyers appointed.
It shall be deemed to constitute a statement of claim.
