Article 975 of the French Code of civil procedure

The statement of appeal shall contain, on pain of nullity:

1° For plaintiffs who are natural persons: an indication of their surname, first names and domicile;

For plaintiffs who are legal persons: an indication of their form, name and registered office and, in the case of administrative or judicial authorities, an indication of their name and the place where they are established;

2° For defendants who are natural persons: an indication of the surname, forenames and domicile;

For defendants who are legal entities: an indication of their form, name and registered office and, in the case of administrative or judicial authorities, an indication of their name and the place where they are established;

3° The constitution of the applicant’s lawyer at the Conseil d’Etat and the Cour de cassation;

4° An indication of the contested decision.

The statement shall specify, where applicable, the heads of the decision to which the appeal is limited.

It shall be signed by the avocat au Conseil d’Etat et à la Cour de cassation.

La déclaration de pourvoi contient, à peine de nullité :

Pour les demandeurs personnes morales : l’indication de leurs forme, dénomination et siège social et, s’agissant des autorités administratives ou judiciaires, l’indication de leur dénomination et du lieu où elles sont établies ;

2° Pour les défendeurs personnes physiques : l’indication des nom, prénoms et domicile ;

Pour les défendeurs personnes morales : l’indication de leurs forme, dénomination et siège social et, s’agissant des autorités administratives ou judiciaires, l’indication de leur dénomination et du lieu où elles sont établies ;

3° La constitution de l’avocat au Conseil d’Etat et à la Cour de cassation du demandeur ;

4° L’indication de la décision attaquée.

La déclaration précise, le cas échéant, les chefs de la décision auxquels le pourvoi est limité.

Elle est signée par l’avocat au Conseil d’Etat et à la Cour de cassation.
