The authorisation decision and the decision authorising its renewal mention:

1° The name of the Maison Sport-Santé;

2° The name and legal form of the legal entity holding the authorisation;

3° The name of the natural person responsible for the activities of the Maison Sport-Santé;

4° The location of the Maison Sport-Santé.

The decision is notified to the applicant and published in the administrative records and on the websites of the regional health agency and the decentralised departments of the Ministry of Sport.

The list of approved sports and health centres is also published on the websites of the regional health agency and the decentralised departments of the Ministry of Sport.

La décision d’habilitation ainsi que celle autorisant son renouvellement mentionnent :

1° Le nom de la maison sport-santé ;

2° Le nom et la forme juridique de la personne morale titulaire de l’habilitation ;

3° Le nom de la personne physique, responsable des activités de la maison sport-santé ;

4° La localisation de la maison sport-santé.

La décision est notifiée au demandeur et publiée au recueil des actes administratifs et sur les sites internet de l’agence régionale de santé et des services déconcentrés du ministère chargé des sports.

La liste des maisons sport-santé habilitées est également publiée sur les sites internet de l’agence régionale de santé et des services déconcentrés du ministère chargé des sports.
