Article D1434-40-1 of the French Public Health Code

The structures mentioned in III of article L. 1434-10 which have taken the initiative to draw up the territorial health project shall send the Director General of the regional health agency with territorial jurisdiction a document specifying the boundaries of the proposed territory and the list of persons or structures involved in drawing it up.

In order to draw up this list, the territorial professional health community and the health, social or medico-social establishment or service initiating the territorial health project shall ask at least the mayors, the presidents of the public establishments for inter-municipal cooperation and the departmental councillors elected in the territory envisaged for the territorial health project, as well as at least one association approved under article L. 1114-1 to take part in drawing it up.

Les structures mentionnées au III de l’article L. 1434-10 ayant pris l’initiative de l’élaboration du projet territorial de santé adressent au directeur général de l’agence régionale de santé territorialement compétent un document précisant la délimitation du territoire proposé ainsi que la liste des personnes ou structures participant à son élaboration.

Pour établir cette liste, la communauté professionnelle territoriale de santé et l’établissement ou service de santé, social ou médico-social qui initient le projet territorial de santé sollicitent au moins les maires, les présidents des établissements publics de coopération intercommunale et les conseillers départementaux élus sur le territoire envisagé du projet territorial de santé ainsi qu’au moins une association agréée au titre de l’article L. 1114-1 pour participer à son élaboration.
