Article D4011-4 of the French Public Health Code

The employment or practice structures declare the implementation of an authorised national protocol to the regional health agency via a dedicated online application on the website of the Ministry of Health and submit the following supporting documents for each member of the voluntary team:

a) A signed and dated commitment agreement;

b) A copy of an identity document;

c) A registration number on the medical register or specific professional file and supporting documents;

d) A sworn statement that they have acquired the skills required to implement the national protocol.

The employment or practice structure must report any changes relating to the members of the team involved in implementing the protocol and provide, at the request of the Regional Health Agency, the documents attesting to the regularity of this implementation.

Each year, the teams involved in a national protocol transmit to the national committee, via a dedicated online application, the data relating to the monitoring indicators and any relevant data mentioned in the protocol. If the implementation of this protocol is suspended by the Regional Health Agency under the conditions laid down in IV of Article L. 4011-3, and if it is not brought into compliance, the establishment will notify it that the protocol is no longer being implemented.

Les structures d’emploi ou d’exercice déclarent la mise en œuvre d’un protocole national autorisé auprès de l’agence régionale de santé via une application en ligne dédiée du site internet du ministère chargé de la santé et déposent, pour chaque membre de l’équipe volontaire, les pièces justificatives suivantes :

a) Accord d’engagement daté et signé ;

b) Copie d’une pièce d’identité ;

c) Numéro d’enregistrement au tableau ordinal ou fichier professionnel spécifique et son justificatif ;

d) Attestation sur l’honneur de l’acquisition des compétences exigées pour la mise en œuvre du protocole national.

La structure d’emploi ou d’exercice signale toute modification relative aux membres de l’équipe engagée dans la mise en œuvre du protocole et fournit à la demande de l’agence régionale de santé les documents attestant de la régularité de cette mise en œuvre.

Les équipes engagées dans un protocole national transmettent annuellement au comité national via une application en ligne dédiée les données relatives aux indicateurs de suivi et toute donnée pertinente mentionnée dans celui-ci. En cas de suspension de la mise en œuvre de ce protocole par l’agence régionale de santé dans les conditions prévues au IV de l’article L. 4011-3, et en l’absence de mise en conformité de celui-ci, l’établissement lui notifie la fin de la mise en œuvre du protocole.
