The contract sets out, for the health care organisation or authorisation holder :
1° Its strategic orientations, the development of its activities and, where appropriate, any related restructuring, taking into account the objectives of the regional health project and those set out in the regional multi-annual plan for risk management and the efficiency of the healthcare system. It also takes account of national public health priorities;

3° The conditions under which patients are received and cared for and the conditions under which continuity of care is guaranteed;
4° Where appropriate, its participation in the emergency care networks mentioned in article R. 6123-26;
5° The organisation enabling it to care for patients referred to it by emergency medicine structures when it holds a medical or surgical care authorisation;
6° The cooperative actions to which it is committed and the procedures for carrying out those which constitute a substantial condition of the authorisation for the healthcare activity or heavy equipment provided for in article L. 6122-1 ;
7° The conditions for the care of patients requiring palliative care;

9° Where applicable, the procedures for implementing teaching and research missions, taking into account the objectives of the four-year contract mentioned in article L. 711-1 of the Education Code;
10° The objectives, together with the monitoring indicators provided for in article R. 6144-2-2 of the Public Health Code, aimed at improving the quality and safety of care;
11° Targets accompanied by monitoring and evaluation indicators designed to improve medical control of expenditure and professional practices. It may provide for accompanying measures aimed at improving management performance, as well as incentives based on results;
12° The conditions for carrying out and evaluating the activities financed under the missions of general interest and contractual support provided for in Article L. 162-22-13 of the Social Security Code, carried out by the health care organisation or the holder of the authorisation provided for in Article L. 6122-1 ;
13° The holder’s commitments in terms of the development of information systems, the transmission of computerised data and, where applicable, telemedicine activities;
14° Where applicable, the procedures for implementing, monitoring and evaluating the missions mentioned in articles L. 6111-1-2 and L. 6111-1-3.
