The application for administrative authorisation provided for in Article L. 6211-2 as it read prior to the entry into force of Order No. 2010-49 of 13 January 2010 as amended relating to medical biology, submitted in the cases described in 1° and 1° bis of III of Article 7 of the said Order, as amended by Law No. 2013-442 of 30 May 2013, shall be sent by registered letter with acknowledgement of receipt to the Director General of the Regional Health Agency of the region in which the medical biology laboratory is established. The application is deemed to be complete if, within two weeks of receipt, the Director General of the Regional Agency has not informed the applicant, by registered letter with acknowledgement of receipt, of the list of missing or incomplete documents.
The applicant must specify the operating conditions of the laboratory and, where applicable, the category or categories of medical biology tests for which authorisation is sought. It must indicate the estimated number of medical biology tests to be performed in the first year of operation, in accordance with the calculation methods defined in article D. 6211-13. The application must be accompanied by documents justifying compliance with the conditions for the proper performance of medical biology analyses determined by the ministerial order provided for in I ofArticle 7 of Order no. 2010-49 of 13 January 2010, as amended, relating to medical biology, as well as the following documentary elements:
1° Description and plan of the premises ;
2° Lists of medical biologists, biologist-responsible or co-responsible biologists and medical laboratory technicians, specifying the full-time equivalence of work for each, as well as their qualifications and diplomas;
3° Social status, where applicable.
Where the operator is not the owner of the equipment or premises, he must indicate how it is used.
