Article D7121-32 of the French Labour Code

The employer shall issue to the employee whom he ceases to employ or who may benefit from his annual leave a certificate justifying his entitlement to leave in duplicate.
This certificate shall indicate:
1° The duration of the engagements or the number of fees completed on behalf of the employer in the preceding twelve months and the amount of remuneration paid during the period in question;
2° The name and address of the paid leave fund to which the employer is affiliated.
It will also provide the employer with a stamped envelope required to send this certificate to the paid leave fund.

L’employeur délivre au salarié qu’il cesse d’employer ou qui peut bénéficier de son congé annuel un certificat justificatif de ses droits à congé en double exemplaire.
Ce certificat indique :
1° La durée des engagements ou le nombre des cachets accomplis pour le compte de l’employeur dans les douze mois qui précédent et le montant de la rémunération versée pendant la période envisagée ;
2° La raison sociale et l’adresse de la caisse de congés payés à laquelle l’employeur est affilié.
Il lui remet également une enveloppe timbrée nécessaire à la transmission de ce certificat à la caisse de congés payés.
