A primary care team is a group of healthcare professionals formed around primary care general practitioners, who choose to carry out their primary care activities as defined in article L. 1411-11 on the basis of a healthcare plan that they draw up. It may take the form of a health centre or a nursing home.
The primary care team contributes to the structuring of healthcare pathways. The aim of its health project is to improve coordination between the various players involved, to prevent, improve and protect the population’s state of health, and to reduce social and territorial inequalities in health.
A specialised care team is a group of healthcare professionals made up of doctors specialising in one or more fields other than general medicine, who choose to coordinate their care activities with all the players in a given area, including primary care teams, on the basis of a healthcare plan that they draw up among themselves.
The specialised care team contributes, along with primary care players, to the structuring of healthcare pathways.
