Article R1111-2 of the French Public Health Code

The applicant may choose to obtain the information requested from the health professional or health institution, either by consulting it on site, with copies of documents being provided where appropriate, or by sending copies of the documents. The cost of providing these copies is to be borne by the applicant under the conditions set out in article L. 1111-7 .

If the information requested is held by a health institution and if the technical facilities of the institution allow this, the applicant may also consult all or part of the information in question electronically.

In the case of a request for on-site consultation addressed to a health care institution, the applicant is informed of the medical support arrangements organised by the institution under the conditions set out in article R. 1112-1.

Copies are drawn up on a medium similar to that used by the health professional or health institution, or on paper, at the choice of the applicant and within the limits of the technical possibilities of the professional or organisation concerned.

Article R1111-2

A son choix, le demandeur obtient du professionnel de santé ou de l’établissement de santé communication des informations demandées, soit par consultation sur place, avec, le cas échéant, remise de copies de documents, soit par l’envoi de copies des documents. Les frais de délivrance de ces copies sont laissés à la charge du demandeur dans les conditions fixées par l’article L. 1111-7.

Dans le cas où les informations demandées sont détenues par un établissement de santé et si les dispositifs techniques de l’établissement le permettent, le demandeur peut également consulter par voie électronique tout ou partie des informations en cause.

Dans le cas d’une demande de consultation sur place adressée à un établissement de santé, le demandeur est informé du dispositif d’accompagnement médical organisé par l’établissement dans les conditions fixées à l’article R. 1112-1.

Les copies sont établies sur un support analogue à celui utilisé par le professionnel de santé ou l’établissement de santé, ou sur papier, au choix du demandeur et dans la limite des possibilités techniques du professionnel ou de l’organisme concerné.
