Article R128-4 of the French Commercial code

I.-A serial number is allocated to each entry.

II.-All entries in the national file of persons prohibited from managing assets include the following:

1° The surname, given name, date and place of birth, nationality and place of residence of the person whose personal bankruptcy has been declared or who is the subject of the prohibition order; the place of residence is that mentioned in the decision declaring the order;

2° The order declared;

3° The court that issued the order;

4° The date on which the order was issued and its duration;

5° Where applicable, the company name or trading name, the address of the registered office and the identity number provided for in Article R. 123-221 of the legal entity of which the person who is the subject of the registration was a de jure or de facto director.

I.-Un numéro d’ordre est attribué à chaque inscription.


II.-Toute inscription au fichier national des interdits de gérer comprend la mention :


1° Des nom de famille, nom d’usage, prénoms, date et lieu de naissance, nationalité et domicile de la personne dont la faillite personnelle a été prononcée ou faisant l’objet de la mesure d’interdiction ; le domicile est celui mentionné dans la décision ayant prononcé la mesure ;


2° De la mesure prononcée ;


3° De la juridiction ayant prononcé la mesure ;


4° De la date du prononcé de la mesure et sa durée ;


5° Le cas échéant, la dénomination ou raison sociale, l’adresse du siège social et le numéro d’identité prévu à l’article R. 123-221 de la personne morale dont la personne qui fait l’objet de l’inscription était dirigeant de droit ou de fait.
