The proposals for staff reports and the staff reports provided for in article R. 15-33-29-28 shall be drawn up on a form conforming to the model laid down by the Minister of Justice.
They shall include a detailed general assessment, as well as a numerical mark from 0 to 10 and an assessment of each of the following elements where they have been observed:
1° Professional relations with the judicial authorities;
2° Quality of the coordination of the department’s judicial police activities;
3° The quality of procedures and the drafting of reports and minutes;
4° Quality of the findings and technical investigations;
> 5° Value of the information provided to the public prosecutor
5° Value of information given to the public prosecutor;
6° Professional commitment;
> 7° Ability to conduct investigations
7° Ability to carry out investigations;
> 8° Degree of confidence granted.

When one of these elements has not been observed, the notation: judicial activity not observed is substituted for the corresponding numerical rating and assessment.
