Article R2132-3 of the French Public Health Code

The doctor who carried out the medical examination will draw up the health certificate corresponding to the child’s age and send it, within eight days, to the doctor in charge of the maternal and child protection service in the department where the parents or the persons exercising parental authority or the persons or services to whom the child has been entrusted reside, in compliance with medical confidentiality.

The results of the examination are recorded in the health record provided for in article L. 2132-1.

The model for health certificates is established by order of the Minister for Health and the Minister for Social Security.

The forms used to draw up the health certificates are attached to the child’s health record which is given to the persons mentioned in article L. 2132-1.

Le médecin qui a pratiqué l’examen médical établit le certificat de santé correspondant à l’âge de l’enfant et l’adresse, dans un délai de huit jours, au médecin responsable du service de la protection maternelle et infantile du département de résidence des parents ou des personnes titulaires de l’exercice de l’autorité parentale ou des personnes ou services à qui l’enfant a été confié, dans le respect du secret médical, et par envoi confidentiel.

Il mentionne les résultats de l’examen dans le carnet de santé prévu à l’article L. 2132-1.

Le modèle des certificats de santé est établi par arrêté du ministre chargé de la santé et du ministre chargé de la sécurité sociale.

Les formulaires destinés à établir les certificats de santé sont annexés au carnet de santé de l’enfant qui est remis aux personnes mentionnées à l’article L. 2132-1.
