Article R2142-25 of the French Public Health Code

The health care organisation must keep, with due regard for confidentiality :

1° A copy of the documents attesting to compliance with the conditions laid down in Article L. 2141-2 ;

2° The written consent of the couple or of the unmarried woman benefiting from medically assisted procreation, given before the procedure is carried out and before the embryo is transferred or before insemination, as well as, in cases where recourse to gametes from a third party donor is necessary, a mention of the date and place of the joint declaration made by the couple in accordance with articles 311-20 of the Civil Code and 1157-2 of the Code of Civil Procedure.

L’établissement de santé doit conserver, dans le respect de leur confidentialité :

1° La copie des pièces attestant du respect des conditions prévues à l’article L. 2141-2 ;

2° Le consentement écrit du couple ou de la femme non mariée bénéficiaire de l’assistance médicale à la procréation, formulé avant la mise en oeuvre de celle-ci et avant le transfert de l’embryon ou avant l’insémination, ainsi que, dans le cas où le recours aux gamètes d’un tiers donneur est nécessaire, la mention de la date et du lieu de la déclaration conjointe du couple prévue aux articles 311-20 du code civil et 1157-2 du code de procédure civile.
