Article R4311-38-2 of the French Public Health Code

The Conseil national de l’ordre registers the service provider on a special list. This registration is exempt from the payment of fees. The Conseil national de l’ordre will send the applicant a receipt stating his registration number, mentioning, where applicable, the speciality or specialities corresponding to the professional qualifications he has declared and, in the case of partial access, the professional title under which he is authorised to practise and the corresponding field of activity, and specifying the competent national health insurance body.

The declaration is renewable every year. In the event of a change in the applicant’s situation as established by the attached documents, the applicant must declare these changes and provide, where applicable, the documents set out in the order referred to in article R. 4311-41-2.

Le Conseil national de l’ordre enregistre le prestataire de services sur une liste particulière. Cet enregistrement est dispensé de cotisation. Le Conseil national de l’ordre adresse au demandeur un récépissé comportant son numéro d’enregistrement, mentionnant, s’il y a lieu, la ou les spécialités correspondant aux qualifications professionnelles qu’il a déclarées et, en cas d’accès partiel, le titre professionnel sous lequel il est autorisé à exercer et le champ d’activités correspondant et précisant l’organisme national d’assurance maladie compétent.

La déclaration est renouvelable tous les ans. En cas de changement de la situation du demandeur telle qu’établie par les documents joints, il déclare ces modifications et fournit, le cas échéant, les pièces fixées par l’arrêté mentionné à l’article R. 4311-41-2.
