The application for authorisation must include :
1° The name and registered office of the company and the address of the establishment;
2° The name and address of the occupational health and safety department to which it reports;
3° The name and position of the radiation protection advisor;
4° The result of the assessment of the risks of exposure to ionising radiation;
5° The circumstances justifying this request, in particular the demonstration that there is no possible alternative to exceeding the exposure limit values;
6° The protective measures and means envisaged;
7° The list of workstations and workers concerned;
8° Where applicable, the special measures taken in the context of work carried out by an external company;
9° The opinion of the occupational physician and the opinion of the social and economic committee.
The employer informs, as appropriate, the Nuclear Safety Authority or the delegate for nuclear safety and radiation protection for defence-related installations and activities.
