The request shall contain all information relevant to the investigation of the claim for compensation, and in particular the indication:
1° The surname, first names, date and place of birth and residence of the claimant;
2° The date, place and circumstances of the offence giving rise to the damage;
3° The offences against the claimant’s person or the damage caused to his or her property;
4° Of the criminal court, if any, before which the offence was brought;
5° Of the family ties or de jure or de facto relationships existing between the claimant and the person who personally suffered the damage if they are not the same person ;
6° Where applicable, the public or private bodies to which the claimant is subject or with which he is insured and which are likely to compensate him for all or part of the loss suffered;
7° Claims for reparation or compensation already made and, in particular, actions for damages that have been brought as well as sums that have already been paid to the claimant as reparation for the loss;
8° The amount of compensation claimed before the commission;
9° The address to which notifications must be made to the claimant.
The claim shall be accompanied by all supporting documents, in particular a copy of the initial medical certificate and, where applicable, all documents attesting to the consolidation, as well as any document enabling the loss or reduction of income, the increase in expenses or the inability to carry out a professional activity that are the consequence of the injury to be assessed.
