A medicinal product, whether simple or compound, held with a view to sale, offered for sale or sold, where one or more of the following particulars have been omitted from one of the components of its packaging, shall be considered to be a secret remedy:
1° For the preparations mentioned in 1° to 3° of article L. 5121-1, the name and address of the pharmacist who dispensed the preparation, except for ampoules and other small primary packaging mentioned in article R. 5121-146-3, for which only the name and postcode may be mentioned;
2° The name and dose of each of the active substances contained in the prepared product.
These last two indications defined in articles R. 5125-58 and R. 5125-59 may be replaced as follows:
a) If it appears in the French pharmacopoeia, or in the form provided for in article R. 5112-4, by the name attributed to the medicinal product in these compendia, followed, where appropriate, by the edition reference ;
b) If the finished product has a poorly defined composition, by the application of the name and qualities of the raw materials used for its preparation as well as the operating procedures followed, the reference and description of the latter must be sufficiently precise to allow, by reproducing them, the obtaining of a remedy with a composition identical to that in question.
