Article R5132-3 of the French Public Health Code

The prescription of medicinal products or products intended for human medicine mentioned in the present section is written, after examination of the patient, on a prescription and indicates legibly :

1° The surname and forenames, position and, where appropriate, the title or speciality of the prescriber as defined in article R. 5121-91, his identifier where available, his business address specifying “France”, his telephone number preceded by the international dialling code “+ 33” and his e-mail address, his signature, the date on which the prescription was written, and for medicinal products subject to hospital prescription or for medicinal products subject to initial hospital prescription, the name of the health establishment or service;

2° The name of the medicinal product or product prescribed, or the active ingredient of the medicinal product designated by its common name, the dosage and instructions for use and, in the case of a preparation, the detailed formula;

3° The duration of treatment or, where the prescription includes the name of the medicinal product within the meaning of article R. 5121-2, the number of packaging units and, where applicable, the number of times the prescription will be renewed;

4° For a medicinal product classified in the category of medicinal products subject to initial hospital prescription, the date on which a new diagnosis is made if the marketing authorisation or temporary use authorisation so provides;

5° The particulars provided for in Article R. 5121-95 and in the eighth paragraph of Article R. 5121-77 where the marketing authorisation or temporary use authorisation so provides;

6° Where applicable, the information provided for in Article R. 5125-54 ;

7° The patient’s full name, sex, date of birth and, if necessary, height and weight;

8° Where applicable, the information required in application of article L. 162-19-1 of the Social Security Code and, where applicable, specified by the registration orders mentioned in articles R. 162-37-2, R. 162-38, R. 163-2, R. 165-1 and R. 165-93 of the same code or by the decision of the Ministers provided for in article R. 163-32 of the same code.

Article R5132-3

La prescription de médicaments ou produits destinés à la médecine humaine mentionnés à la présente section est rédigée, après examen du malade, sur une ordonnance et indique lisiblement :

1° Les nom et prénoms, la qualité et, le cas échéant, le titre, ou la spécialité du prescripteur telle que définie à l’article R. 5121-91, son identifiant lorsqu’il existe, son adresse professionnelle précisant la mention ” France “, ses coordonnées téléphoniques précédées de l’indicatif international ” + 33 ” et son adresse électronique, sa signature, la date à laquelle l’ordonnance a été rédigée, et pour les médicaments à prescription hospitalière ou pour les médicaments à prescription initiale hospitalière, le nom de l’établissement ou du service de santé ;

2° La dénomination du médicament ou du produit prescrit, ou le principe actif du médicament désigné par sa dénomination commune, la posologie et le mode d’emploi, et, s’il s’agit d’une préparation, la formule détaillée ;

3° La durée de traitement ou, lorsque la prescription comporte la dénomination du médicament au sens de l’article R. 5121-2, le nombre d’unités de conditionnement et, le cas échéant, le nombre de renouvellements de la prescription ;

4° Pour un médicament classé dans la catégorie des médicaments à prescription initiale hospitalière, la date à laquelle un nouveau diagnostic est effectué lorsque l’autorisation de mise sur le marché ou l’autorisation temporaire d’utilisation le prévoit ;

5° Les mentions prévues à l’article R. 5121-95 et au huitième alinéa de l’article R. 5121-77 lorsque l’autorisation de mise sur le marché ou l’autorisation temporaire d’utilisation les prévoit ;

6° Le cas échéant, la mention prévue à l’article R. 5125-54 ;

7° Les nom et prénoms, le sexe, la date de naissance du malade et, si nécessaire, sa taille et son poids ;

8° Le cas échéant, les éléments requis en application de l’article L. 162-19-1 du code de la sécurité sociale et éventuellement précisés par les arrêtés d’inscription mentionnés aux articles R. 162-37-2, R. 162-38, R. 163-2, R. 165-1 et R. 165-93 du même code ou par la décision des ministres prévue à l’article R. 163-32 du même code.
