Article R5142-57 of the French Public Health Code

In establishments authorised to manufacture and import medicated feed, the following information is recorded:

1° The date of manufacture, import, transfer or issue as appropriate and, if manufacture has been subcontracted, the date of the manufacturing order and the date of receipt of the batch;

2° The name, quantity and batch number of the medicinal premix or premixes used;

3° The nature and quantity of the feed used;

4° The trade name or, failing that, the nature and quantity of the medicated feed manufactured, imported, held and transferred;

5° The batch number of the medicated feed and the expiry date;

6° Where applicable, the name and address of the veterinarian who issued the prescription and the name and address of the addressees.

Article R5142-57

Dans les établissements autorisés à fabriquer et à importer des aliments médicamenteux, les renseignements suivants sont consignés :

1° La date de la fabrication, de l’importation, de la cession ou de la délivrance suivant le cas ainsi que, si la fabrication a été sous-traitée, la date de l’ordre de fabrication et la date de réception du lot ;

2° La dénomination, la quantité et le numéro de lot du ou des prémélanges médicamenteux utilisés ;

3° La nature et la quantité des aliments utilisés ;

4° La dénomination commerciale ou, à défaut, la nature ainsi que la quantité de l’aliment médicamenteux fabriqué, importé, détenu et cédé ;

5° Le numéro de lot de l’aliment médicamenteux et la date de péremption ;

6° Selon le cas, le nom et l’adresse du vétérinaire qui a établi la prescription ainsi que le nom et l’adresse des destinataires.
