Article R5212-39 of the French Public Health Code

Each user department of a medical device on the list provided for in Article R. 5212-36 completes the information mentioned in Article R. 5212-38 by recording:

-the date of use ;

the identification of the patient, and in particular his surname, first name and date of birth; – the name of the doctor or surgeon;

-the name of the user doctor or dental surgeon.

Chaque service utilisateur d’un dispositif médical figurant sur la liste prévue à l’article R. 5212-36 complète les informations mentionnées à l’article R. 5212-38 en enregistrant :

-la date d’utilisation ;

-l’identification du patient, et notamment ses nom, prénom et date de naissance ;

-le nom du médecin ou du chirurgien-dentiste utilisateur.
