Article 1203 of the French Code of civil procedure

The matter is referred to the court or judge by petition delivered or addressed to the clerk of the court’s office. Except in the case of applications for delegation of parental authority, the parties are required to instruct a lawyer. The application may be addressed to the public prosecutor, who must forward it to the court or judge.

In addition to the information provided for in article 57, the application shall indicate, on pain of inadmissibility, the place where the minor lives and, where applicable, the place where the holder or holders of parental authority live, as well as the grounds for the application.

Article 1203

Le tribunal ou le juge est saisi par requête remise ou adressée au greffe. Sauf pour les demandes de délégation de l’autorité parentale, les parties sont tenues de constituer avocat. La requête peut être adressée au procureur de la République qui doit la transmettre au tribunal ou au juge.

Outre les mentions prévues à l’article 57, la requête indique, à peine d’irrecevabilité, le lieu où demeure le mineur et, le cas échéant, le lieu où demeurent le ou les titulaires de l’autorité parentale ainsi que les motifs de la requête.
