Article 790 of the French Code of Criminal Procedure

The convicted person shall address the application for rehabilitation to the public prosecutor of his current residence or, if he lives abroad, to the public prosecutor of his last residence in France or, failing that, to the public prosecutor of the place of conviction.

This application shall specify:

1° The date of the conviction;

2° The places where the convicted person has resided since his release.

Le condamné adresse la demande en réhabilitation au procureur de la République de sa résidence actuelle ou, s’il demeure à l’étranger, au procureur de la République de sa dernière résidence en France ou, à défaut, à celui du lieu de condamnation.

Cette demande précise :

1° La date de la condamnation ;

2° Les lieux où le condamné a résidé depuis sa libération.
