WORK PLACEMENT AGREEMENT

Article 1

This agreement has been drawn up between:
1. the teaching or training structure approved by the regional director of youth, sport and community life (name of the structure), represented by its president or director, Mr;
2. the head of the establishment or department responsible for the training (address).

Article 2

(Name of the teaching or training establishment) undertakes to give Mr
(surname and forenames of the trainee) the opportunity to undertake, as part of his training and preparation for the first-degree state diploma for sports instructors (option),
for

, a work-based teaching placement, in application of articles A. 212-134, A. 212-135 and A. 212-136 .

Article 3

The role of the training advisor M. is:
-to prepare the trainee for his or her future activity as a teacher or coach of (specify the sport);
-to assess the trainee’s behaviour as an educator;
-to advise the trainee on technical and pedagogical matters in compliance with the ethical rules of the sport;
-to help the trainee complete his or her training;
-to write the end-of-training report, which is included in the training record booklet.
As part of this training, the teaching or training structure undertakes to:
– provide a weekly assessment with the trainee;
– keep the training logbook.

Article 4

Mr (full name)
undertakes to comply with the internal rules of the teaching or training facility and to take an active part in the training provided.

Article 5

The teaching and training structure, the consultant and the trainees must be insured for civil liability in application of articles 1240 and 1242 of the French Civil Code.

Article Annexe II-9 (art. A212-136)

CONVENTION DE STAGE PÉDAGOGIQUE EN SITUATION

Article 1er

La présente convention est établie entre :
1. La structure d’enseignement ou d’entraînement agréée par le directeur régional de la jeunesse, des sports et de la vie associative (dénomination de la structure), représentée par son président ou directeur, M. ;
2. Le chef de l’établissement ou du service responsable de la formation (adresse).

Article 2

(Dénomination de la structure d’enseignement ou d’entraînement) s’engage à donner à M.
(nom et prénoms du stagiaire) la possibilité d’effectuer, à titre de formation et de préparation au brevet d’Etat d’éducateur sportif du premier degré (option),
pendant
heures (à raison de
heures/ semaine) à dater du
un stage pédagogique en situation, en application des articles A. 212-134, A. 212-135 et A. 212-136.

Article 3

Le conseiller de stage M. a pour rôle :
-de préparer le stagiaire à sa future activité d’enseignant ou d’entraîneur de (préciser la discipline sportive) ;
-d’apprécier son comportement d’éducateur ;
-de conseiller le stagiaire dans les domaines technique et pédagogique dans le respect des règles déontologiques de la discipline sportive ;
-de l’aider à compléter sa formation ;
-de rédiger le rapport de fin de stage qui est reporté sur le livret de formation.
La structure d’enseignement ou d’entraînement s’engage, dans le cadre de cette formation, à assurer :
-un bilan hebdomadaire avec le stagiaire ;
-la tenue du livret de formation.

Article 4

M. (nom et prénoms)
s’engage à respecter le règlement intérieur de la structure d’enseignement ou d’entraînement et à participer activement à la formation qui lui est dispensée.

Article 5

La structure d’enseignement et d’entraînement, le conseiller et les stagiaires doivent être assurés en responsabilité civile en application des articles 1240 et 1242 du code civil.
