Article D4151-25 of the French Public Health Code

I.-Midwives may prescribe and administer the vaccines mentioned in the decrees provided for in 1° and 2° of article L. 4151-2 to persons whose age conditions and, where applicable, pathologies are specified by these same decrees.

II – The midwife shall enter in the health record, vaccination record or shared medical record of the person vaccinated, his or her full name, the name of the vaccine administered, its batch number and the date it was administered. If this information is not recorded, it will issue the vaccinated person with a vaccination certificate containing this information.

In the absence of a shared medical file and subject to the consent of the person vaccinated, it will send this information to the person’s GP. This transmission is carried out by secure messaging system meeting the conditions laid down in article L. 1470-5, where such a system exists.

I.-La sage-femme peut prescrire et administrer les vaccins mentionnés dans les arrêtés prévus aux 1° et 2° de l’article L. 4151-2 aux personnes dont les conditions d’âge et, le cas échéant, les pathologies sont précisées par ces mêmes arrêtés.

II.-La sage-femme inscrit dans le carnet de santé, le carnet de vaccination ou le dossier médical partagé de la personne vaccinée, ses noms et prénoms d’exercice, la dénomination du vaccin administré, son numéro de lot et la date de son administration. A défaut de cette inscription, elle délivre à la personne vaccinée une attestation de vaccination qui comporte ces informations.

En l’absence de dossier médical partagé et sous réserve du consentement de la personne vaccinée, elle transmet ces informations au médecin traitant de cette personne. Cette transmission s’effectue par messagerie sécurisée répondant aux conditions prévues à l’article L. 1470-5, lorsqu’elle existe.
