Article D4153-5 of the French Labour Code

Any employer intending to employ a minor must submit a written request to the Labour Inspector at least fifteen days before the planned date of recruitment.
The request must include:
1° The surname, first names, age and place of residence of the person concerned;
2° The duration of the employment contract;
3° The nature and conditions of work envisaged;
4° The working hours;
5° The amount of remuneration;
6° The written and signed agreement of the person’s legal representative.

L’employeur qui envisage d’employer un mineur adresse une demande écrite à l’inspecteur du travail au moins quinze jours avant la date prévue d’embauche.
La demande comporte :
1° Les nom, prénoms, âge et domicile de l’intéressé ;
2° La durée du contrat de travail ;
3° La nature et les conditions de travail envisagées ;
4° L’horaire de travail ;
5° Le montant de la rémunération ;
6° L’accord écrit et signé du représentant légal de l’intéressé.
