1° Relies on inter-hospital multidisciplinary referral teams for the care of these patients. These multidisciplinary teams are set up by cancer treatment authorisation holders, if necessary in conjunction with the specific regional cancer system and the inter-regional paediatric oncology referral hospital organisation.
2° Ensures that the transition phase in the care of adolescents with cancer towards adult oncology care is organised by the paediatric oncology and adult oncology care teams concerned.
