Article D6124-198 of the French Public Health Code

I.-The holder of the home hospital care authorisation appoints, from among the members of the multidisciplinary team, a coordination team comprising at least:

1° A practising home hospital doctor;

2° A State-qualified or registered nurse;

3° A social service assistant, a social and family economics advisor or a socio-educational assistant.

II.- The coordination team coordinates the care provided to patients in liaison with the health structures and professionals involved before and after the home hospital stay.The co-ordination team co-ordinates the care given to patients in conjunction with the structures and healthcare professionals involved upstream and downstream of the home hospital stay.

III – The members of the coordination team are staff who report directly to the authorisation holder.

I.-Le titulaire de l’autorisation d’activité de soins d’hospitalisation à domicile désigne, parmi les membres de l’équipe pluridisciplinaire, une équipe de coordination comportant au moins :

1° Un médecin praticien d’hospitalisation à domicile ;

2° Un infirmier diplômé d’Etat ou autorisé ;

3° Un assistant de service social, un conseiller en économie sociale et familiale ou un assistant socio-éducatif.

II.-L’équipe de coordination assure la coordination des soins dispensés aux patients en lien avec les structures et professionnels de santé intervenant en amont et en aval du séjour en hospitalisation à domicile.

III.-Les membres de l’équipe de coordination sont des personnels relevant directement du titulaire de l’autorisation.
