I.-The Hospitalisation at Home doctor organises the medical operation of the facility, in accordance with its medical project where applicable. He ensures that the care and services provided to patients are appropriate and continuous, and that the medical records required for continuity of care are transmitted. He gives his opinion on the admission and discharge of patients.
II – When the doctor treating the patient, or failing this the doctor designated by the patient, is unable to ensure continuity of care through an intervention at home or a telehealth activity, under the conditions and within the timescales required by changes in the patient’s situation, this continuity is ensured, including with regard to prescriptions, by the hospital at home doctor or the doctor on call for the holder of the authorisation for the hospital at home care activity.
