Article D6124-446 of the French Public Health Code

The director of the establishment must keep :

1° A special register signed by the doctor assigned to the establishment on each visit. All his observations, all medical incidents and all his prescriptions are recorded in this register; the observations of the medical inspectors and the officials responsible for monitoring are also recorded in this register;

2° A register in which the surname, first names, date and place of birth of each mother and each child, the date of admission and the date and reason for discharge are recorded. On this register, each mother and each child has a number which is reproduced on the observation sheets;

3° The observation sheets, on which the dates of admission and discharge are recorded, as well as any incidents that occurred during the child’s stay in the establishment, medical observations and treatments received, are kept in the establishment for inspection by public health inspectors;

4° A notebook of food preparations and daily menus.

Le directeur de l’établissement doit tenir :

1° Un registre spécial sur lequel le médecin affecté à l’établissement appose sa signature à chacune de ses visites. Sur ce registre, sont consignées toutes ses observations, tous les incidents d’ordre médical, ainsi que toutes ses prescriptions ; y sont consignées également les observations des médecins inspecteurs et des fonctionnaires chargés du contrôle ;

2° Un registre sur lequel sont inscrits les nom, prénoms, date et lieu de naissance de chaque mère et de chaque enfant, la date de l’admission, la date et le motif de la sortie. Sur ce registre, chaque mère et chaque enfant a un numéro qui est reproduit sur les feuilles d’observations ;

3° Les feuilles d’observations, sur lesquelles sont consignées les dates d’entrée et de sortie, les incidents survenus au cours du séjour dans l’établissement, l’observation médicale et les traitements reçus sont conservés dans l’établissement et permettent le contrôle aux médecins inspecteurs de santé publique ;

4° Un carnet des préparations alimentaires et des menus quotidiens.
