As part of its general interest activities, the complex healthcare pathway coordination support scheme provides a multi-purpose service to any professional who requests it, to carry out the tasks mentioned in 1° and 2° of article L. 6327-2, in order to offer the person being cared for a comprehensive and coordinated response, regardless of their age, pathology, disability or situation.
These tasks are carried out in consultation with the attending physician.
It participates in the territorial coordination of players, in particular by analysing needs and structuring complex healthcare pathways, by supporting inter-professional practices and by supporting professional initiatives.
The person concerned is informed of the use of the coordination support system so that they can exercise their right to object.
