Article D6327-2 of the French Public Health Code

Each coordination support system has a single information system shared between the professionals involved in the system. It enables the exchange and sharing of information concerning the same person being cared for between professionals working within the coordination support system and with third-party professionals working with the person as part of the care team defined in article L. 1110-4.

The information system of the coordination support system meets the security and interoperability requirements laid down in article L. 1110-4-1 and is part of the strategy defined by the regional health agency in application of article L. 1431-2.

Chaque dispositif d’appui à la coordination dispose d’un système d’information unique partagé entre les professionnels intervenant dans le dispositif. Il permet l’échange et le partage d’informations concernant une même personne prise en charge entre professionnels exerçant au sein du dispositif d’appui à la coordination et avec les professionnels tiers intervenant auprès de la personne dans l’équipe de soins définie par l’article L. 1110-4.

Le système d’information du dispositif d’appui à la coordination répond aux exigences de sécurité et d’interopérabilité prévues par l’article L. 1110-4-1 et s’inscrit dans la stratégie définie par l’agence régionale de santé en application de l’article L. 1431-2.
