For group life insurance contracts mentioned in article L. 141-1 with surrender or transfer values, where the relationship between the subscriber and the policyholder does not make membership of the contract compulsory, the information sheet provided by the policyholder includes, in addition to the information mentioned in the second paragraph of article L. 141-4, that contained in the memorandum mentioned in article L. 132-5-2. The box mentioned in the first paragraph of article L. 132-5-2 is inserted at the beginning of the notice. When the policy is taken out, the policyholder must provide the member with the model form referred to in the third paragraph of article L. 132-5-2. The policyholder must be informed of the information referred to in the fourth paragraph of the same article and, under the conditions defined in the same article, of the surrender or transfer values. The waiver option is exercised in accordance with articles L. 132-5-1 and L. 132-5-2.
The prospectus must indicate the corporate purpose and contact details of the subscriber.
The information sheet specifies that the rights and obligations of the member may be modified by amendments to the said contracts. The terms and conditions for the adoption of these riders by the policyholder are communicated by the policyholder to the member.
The policyholder provides the member with the information drawn up by the insurance undertaking under the same conditions as those set out in article L. 132-22.
