Article L321-6 of the French Sports Code

Where the approved federation to which the sports association is affiliated offers its members, who apply for a licence, the option of simultaneously taking out a group personal insurance policy underwritten by the federation, it is obliged :

1° Formulate this proposal in a document, which may or may not be separate from the licence application, which mentions the cost of membership, specifies that membership is not compulsory and indicates that the member of the group contract may also take out additional individual cover;

2° To attach to this document a notice drawn up by the insurer in accordance with the second paragraph of article L. 141-4 of the Insurance Code.

Lorsque la fédération agréée à laquelle est affiliée l’association sportive propose aux membres de celle-ci, qui sollicitent la délivrance d’une licence, d’adhérer simultanément au contrat collectif d’assurance de personnes qu’elle a souscrit, elle est tenue :

1° De formuler cette proposition dans un document, distinct ou non de la demande de licence, qui mentionne le prix de l’adhésion, précise qu’elle n’est pas obligatoire et indique que l’adhérent au contrat collectif peut en outre souscrire des garanties individuelles complémentaires ;

2° De joindre à ce document une notice établie par l’assureur conformément au deuxième alinéa de l’article L. 141-4 du code des assurances.
