Applications for entry on the roll are sent by the interested parties to the competent regional council of the ordre. They must be accompanied by a dossier, the composition of which is laid down by decree in the Conseil d’Etat.
In the event of cessation or modification of the professional activity or change of address of the establishment, a declaration, accompanied by the necessary supporting documents, is sent within fifteen days to the competent council of the ordre which, if necessary, amends the registration or removes the name, followed, where appropriate, by a new entry on the roll, in the light of the documents sent.
By way of derogation from the last paragraph of article L. 4231-1, a pharmacist who interrupts his activity for a period of less than one year and who does not carry out any other activity during this interruption is omitted from the roll by the competent Association Council. The period of omission may be renewed, but may not exceed a total period of two years. The conditions under which a council proceeds with omission are defined by decree.
